Urology is one of the most visited specialties in the Philippines β yet many patients delay their appointment for months, often out of embarrassment or the belief that their symptoms will resolve on their own. Kidney stones, prostate problems, and urinary tract infections are among the most common conditions treated by Filipino urologists, and early intervention almost always leads to better outcomes.
Here are seven signs it's time to book an appointment.
1. Blood in Your Urine (Hematuria)
Seeing pink, red, or dark brown urine is never normal and is always worth investigating. Hematuria can have many causes β from a simple urinary tract infection (UTI) to kidney stones, bladder polyps, or, in some cases, bladder or kidney cancer.
Even a single episode of visible blood in the urine warrants a urology evaluation, particularly in men over 40 and smokers.
2. Kidney Stones
The Philippines has a relatively high prevalence of urolithiasis (kidney stones), partly due to diet (high salt and meat intake) and inadequate water consumption. Symptoms include:
- Sudden, severe pain in the back, side, or lower abdomen (renal colic)
- Pain that radiates to the groin
- Nausea and vomiting
- Frequent, painful urination
- Blood in the urine
Small stones may pass on their own with increased fluid intake. Larger stones may require ESWL (shock wave lithotripsy), ureteroscopy, or surgery. A urologist will determine the best approach based on stone size and location via KUB ultrasound or CT scan.
3. Difficulty Urinating or Weak Stream (Men)
Filipino men over 50 frequently experience lower urinary tract symptoms (LUTS) related to an enlarged prostate β also called Benign Prostatic Hyperplasia (BPH). Signs include:
- A weak or interrupted urine stream
- Difficulty starting urination
- Feeling that your bladder doesn't empty completely
- Frequent urination, especially at night (nocturia)
- Urgent need to urinate
BPH is very common and very treatable. Don't wait until you can't urinate at all β that's a medical emergency. A urologist can manage BPH with medications (alpha-blockers, 5-alpha-reductase inhibitors) or minimally invasive procedures.
4. Recurrent Urinary Tract Infections (UTIs)
UTIs are common in women, but recurrent UTIs (three or more per year) signal that something else may be going on β a structural abnormality, incomplete bladder emptying, or an underlying condition that makes you more susceptible. In men, any UTI is unusual and warrants urological evaluation.
A urologist can identify the underlying cause and recommend targeted treatment to break the cycle.
5. Prostate Cancer Screening (Men Over 50)
Prostate cancer is the most common cancer in Filipino men and the third leading cause of cancer death among males in the country. It is often asymptomatic in the early stages, making screening critical.
The Philippine Urological Association recommends that men discuss PSA (Prostate-Specific Antigen) testing with their doctor starting at age 50 β or age 40β45 if they have a family history of prostate cancer. A urologist is the right specialist for PSA interpretation and any subsequent prostate biopsy.
6. Male Infertility
About 40β50% of infertility cases involve a male factor. A urologist β specifically one with subspecialty training in male reproductive medicine β evaluates conditions such as:
- Low sperm count (oligospermia) or absence of sperm (azoospermia)
- Varicocele (dilated veins in the scrotum that can impair sperm production)
- Erectile dysfunction and ejaculatory disorders
- Hormonal imbalances affecting fertility
If you and your partner have been trying to conceive for over a year without success, both partners should be evaluated simultaneously.
7. Incontinence (Urine Leakage)
Urinary incontinence β the involuntary leakage of urine β affects both men and women and is significantly underreported in the Philippines because patients are embarrassed to discuss it. Causes range from weak pelvic floor muscles after childbirth, to prostate surgery side effects, to overactive bladder syndrome.
A urologist can identify the type of incontinence (stress, urge, overflow) and recommend pelvic floor rehabilitation, medication, or surgical correction.
What to Expect at Your First Urology Visit
Your urologist will take a detailed history and may request:
- Urinalysis and urine culture
- KUB ultrasound (kidneys, ureter, bladder)
- PSA blood test (for men with prostate concerns)
- Uroflowmetry (measuring urine stream speed and volume)
- Cystoscopy in certain cases
